Right Superficial Femoral Artery Lesion Treated by Stent implantation

Case Presentation
The patient was a 58 year-old man with intermittent claudication for several years. His cardiovascular risk factors were hypertension, dyslipidemia and smoking. He was diagnosed with peripheral artery disease (Right superficial femoral artery) by low extremity CT at Asan medical center.
Baseline Angiography
Baseline lower-limb angiography showed a diffuse total occlusion at the right superficial femoral artery (SFA) ( Movie 1).
Procedural Step
A 7Fr sheath was inserted into the left femoral artery. And then, we exchanged it for the Shuttle introducer sheath. We inserted 0.035-inch Terumo stiff wire, and it passed easily the total occluded lesion. Pre-ballooning with Mustang 4.0 x 150 mm was performed at right SFA (Figure 1). After that, stent implantation was done using self-expandable Epic stent 6.0 x 120mm. However, there was residual stenosis about 30% after stenting. Additional ballooning with Mustang 5.0 x 180mm was performed at lesion site. And then, another overlapping Epic stent 6.0 x 120mm was deployed ( Movie 2). Post-deployment balloon dilatation was performed using Mustang 5.0 x 180mm, sequentially (Figure 2). Final angiogram showed well-positioned stent in Right SFA ( Movie 3).
