Percutaneous Endovascular Repair of Abdominal Aortic Aneurysm Using Endovascular Stent Graft System

Relevant clinical history and physical examination
A 72 years old woman visited to local hospital with intermittent flank pain. AAA was detected accidentally by AP CT. So, she was referred to our hospital for endovascular aneurysm repair of known abdominal aortic aneurysm. y ear old woman was admitted with increasing size of abdominal aortic aneurysm (44.2 ¡æ 49.5 mm for 1year). She had been treated with coronary artery disease and hypertension.
Relevant test results prior to catheterization
A CT scan showed 49.5 mm sized, fusiform shaped infrarenal aortic aneurysm. (Figure 1)
Procedure
Both femoral arteries were exposed by cut down procedure and were punctured with 8 Fr sheath. With pigtail catheter, aortography was done. ( Movie 1) After 0.035 inch stiff wire insertion, Endurant stent graft system 36X16X166 was deployed via Lt. femoral approach. ( Movie 2, Movie 3) After stent insertion, Powerflex balloon (10 Fr, 40 mm sized) was inflated into in-stent area. (Figure 2) Endurant stent graft system 16X20X93 was deployed at Rt. CIA. ( Movie 4) And then another Endurant stent graft system 16X20X82 was deployed at distal Rt. CIA. ( Movie 5) High pressure balloon dilatation was done with Powerflex balloon (10 Fr, 40 mm sized). Final angiogram showed successful exclusion of AAA. ( Movie 6)
