Clinical Information

- Relevant clinical history and physical exam:
A 55-year-old man was presented with intermittent claudication (200~300m) for 2 years. CT angiogram revealed significant stenosis at both lower extremity arteries (Figure 1). Two months ago, revascularization was performed at right iliac and superficial femoral arteries. He was admitted again to undergo PTA at left superficial femoral artery.

- Relevant test results prior to catheterization:
Ankle-brachial index : right 0.13 (before treatment), left 0.27
Segmental limb pressure : Pressure drop was noted at right aorto-iliac and left femoro-popliteal segments.

Interventional Management
- Procedural step:
A 7 Fr sheath was inserted into the right femoral artery. We crossed lesion using Terumo wire. Then, percutaneous transluminal angioplasty was performed using a 5.0 x 40 mm Powerflex balloon (Figure 2). Next, SMART stent (6.0 x 100 mm) were successfully deployed (Figure 3). Good patency was achieved (Figure 4).
