  • carlos fernandez pereira 2003-07-31 It was an excellent result, but I think we should treat with balloon first,to restore the flow ;then usually we are going to have LDA disease and probably also treat with balloon and stent and then at few moments after ,put a stent in Left main.When we see a Left main occluded totally in AMI and the patient is still alive,it is because we have collaterals from RCA.From 1995 we have 148 cases of unprotected left main disease, of them 19 were in AMI,and all received abciximab.The other thing in elective cases is that probably most of the cases we are going to find other vessels to treat,but we prefer to treat LCX in second time and avoid if possible T stent with bare stents.Now we have the advantage of DES stents to compete with surgeons,and maybe it is one of the indications in LM. sincerely yours Carlos Fernandez Pereira MD Otamendi Hospital Buenos Aires Argentina
  • Seung-Jung Park 2003-08-05 I fully agree with your opinion. Actually, we didn't describe the detail in method. We did predilation with balloon before the stenting procedure in almost all cases except 2 patients who had the isolated left main shaft disease. In the era of DES, the unprotected left main stenting would be more acceptable in selected patients group. Thanks for your comments.
